This consult provides the opportunity to spend the necessary time to deliver more comprehensive patient care with a focus on wellness.

I perform an in depth nursing assessment to determine the direction of care together with you that may include the following:

• Blood Pressure and vital signs, reviewing related medications.
• Discuss Diabetic concerns about disease, testing or medications used.
• Wound Care assessing skin integrity and healing strategies as well as prevention of breakdown.
• Develop a personalized care plan considering all avenues in Alternative and mainstream medicine when chronic issues are present.
• Develop a care plan in the prevention of chronic illness within our network of care to support your lifelong journey to wellness.
